Shopify app
Workflow Forms
Build contact, newsletter, request or feedback forms from a reusable field library and show them inline, as a popup or on a direct link. Every submission is stored in an inbox and fires the Form submitted trigger in Shopify Flow with all its answers.
How it works
Storefront forms that start Shopify Flow workflows
1. Build the form
Pick fields from the library, set conditions, write the success message and choose the branding. Translations live next to the form.
2. Show it in the store
Add the theme block to any page in the theme editor, turn on the popup app embed or share the direct link page.
3. Let Shopify Flow do the rest
Email your team, tag the customer, create a draft order or call another system. Ready-made templates get you started.
Live demos
Three ways to show a form
All three run in this store. Send one and it starts a Shopify Flow workflow that emails our team.
Inline theme block
A contact form placed on a page with the theme editor. No code, styled by the app's branding.
Popup
A newsletter form that opens after a few seconds and remembers when a visitor closed it. Enabled with one app embed.
Direct link page
A multi-step wholesale request on its own page, served by the app on this domain. Share the link anywhere.

Questions
About Workflow Forms
More answers in the documentation, or ask us at support@codecreationlabs.com.
Does the visitor need an account?
No. Forms work for every visitor. If the visitor is logged in, the submission carries their customer ID so a workflow can tag or update the customer.
Can the form match my theme?
Colors, fonts and shape come from the branding settings, and the theme block can override them per placement. Custom CSS and Liquid are available for the rest.
Where are submissions stored?
In the app's inbox, encrypted, on Google Cloud in Belgium. They stay until you delete them, and you can export them as CSV or copy them to metaobjects.
